New Email Address - What's Required

If you need a new e-mail address to be created, please submit your request with the following information included:

1. The required address (obvious, but it's surprising how often people forget it)
2. Is it for an existing account, or a new account?
3. Will it be for an individual user, or a shared mailbox?
4. If it's shared, who needs access to it?
5. Will e-mail be sent from the address?
